Services

Interested in attending a Day Program or moving into one of the Residential Programs in our area? Here's what you need to do:

  • First - Contact your Case Manager and inform them of your need or desire to attend a Day Program or move into a residence. (How do I get a Case Manager?)
  • Second- Your Case Manager will discuss your needs or desires. Your support plan will be amended, if necessary. Your Case Manager will offer you a choice of providers from the Qualified Provider List.
  • Third - Once you chosen your provider, your Case Manager will contact the provider and work to have you enrolled.

Remember, Services are based on need and availability of resources. A consumer could be in need of many services and supports. However, it may not be possible to receive the service or support needed because of waiting lists or funding constraints.

There are currently waiting lists for some day/vocational services.
There are waiting lists for residential services.
